چکیده
In the present case, we describe the abrupt transformation of intra-pulmonary vein activity from rapid firing to dissociated ectopic activity during sinus rhythm, as an easily identifiable, though rare to encounter, sign which documents the achievement of bidirectional block.

Usefulness of filtered unipolar electrogram morphology for evaluating transmurality of ablated lesions during pulmonary vein isolation
BACKGROUND Although alteration of the amplitude and morphology of bipolar electrograms is used widely as a guide of the ablation effect, there is little information concerning unipolar electrograms. The amplitude and morphology of filtered bipolar (BP) and filtered unipolar (UP) electrograms were compared during pulmonary vein isolation in patients with atrial fibrillation. METHODS BP (30-250...
متن کاملElectrophysiological Evaluation of Pulmonary Vein Isolation.
Since the pulmonary veins (PVs) were identified as a major source of AF triggers, ablation strategies targeting the PVs have evolved from focal ablation inside the PVs to wide area circumferential PV isolation (PVI) which at this juncture is the standard approach. Despite the widespread popularity of PVI, a universal definition is lacking. While "entrance block" is a generally accepted endpoint...
متن کاملConducting randomized trials in the electrophysiology laboratory: lessons from a randomized comparison of recording methods during pulmonary vein isolation by segmental ostial ablation.
Accurate identification of pulmonary vein (PV) potentials during segmental ostial ablation for PV isolation is very important to completely isolate the PVs and also to avoid unnecessary applications of radiofrequency energy. A prior post hoc analysis of unipolar and bipolar electrograms recorded from successful and unsuccessful ablation sites demonstrated that unipolar electrograms recorded at ...
متن کاملLessons from dissociated pulmonary vein potentials: entry block implies exit block
AIMS Prior reports using pacing manoeuvres, demonstrated an up to 42% prevalence of residual pulmonary vein to left atrium (PV-LA) exit conduction after apparent LA-PV entry block. We aimed to determine in a two-centre study the prevalence of residual PV-LA exit conduction in the presence of unambiguously proven entry block and without pacing manoeuvres. METHODS AND RESULTS Of 378 patients, 1...
متن کاملPulmonary vein isolation: comparison of bipolar and unipolar electrograms at successful and unsuccessful ostial ablation sites.
INTRODUCTION No prior studies have quantitatively analyzed the characteristics of bipolar or unipolar electrograms that may be helpful in identifying successful ostial ablation sites in patients with atrial fibrillation undergoing segmental pulmonary vein isolation. METHODS AND RESULTS The characteristics of bipolar and unipolar electrograms recorded at 185 successful and 120 unsuccessful ost...
